Distribution: Occurring on both sides of the Cascades in Washington; British Columbia to California, east along the U.S. and Canadian border to the Atlantic coast.
Habitat: Moist humus in shady coniferous and deciduous forests, low to mid-elevations.

Myco-heterotrophic, scapose, perennial herbs, the stems 1.5-4 dm. tall, purplish-tinged.
Plant has no true, green leaves; scape with several sheathing bracts.
Flowers 7-30 on pedicles 1-3 mm. long in a raceme up to 2 dm. long; sepals 3, oblong-lanceolate, 10-17 mm. long and 2-4 mm. broad, pink or yellowish-pink with 3 narrow, reddish-brown stripes; 2 small petals similar, slightly shorter and broader; third petal (lip) broadly oblong-elliptic, not lobed, 8-15 mm. long and 6-8 mm. broad, concave, with upturned margins, with deep-colored stripes; stamens and style fused to form a column 6-7 mm. long, purple-spotted near the base.
Capsule pendant, ovoid, 1.2-2.5 cm long.
